Appointments
Please call the office to make an appointment in advance. If you're unable to
keep your appointment, please call us as far in advance as possible so we may
use that time to see another patient in need of care. We make a sincere effort
to adhere to our appointment schedule and appreciate your patience if we are
late due to emergencies or hospital surgery.
Form Provided below is a form that requires completion prior
to your visit and/or surgery. Please download the form as directed
by your physician and/or staff by clicking on the name of the form. If

you to download this software, providing access to the form. Please bring
the completed form with you to your scheduled visit. This will help expedite

Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore,
we request payment arrangements for all office services at the time they are
rendered
unless prior arrangements have been made. We accept Visa and Mastercard.
If we are a participating provider of your insurance company, we'll bill them.
However,
payment is the
patient's
responsibility. We'll help in any way we can to assist you in handling
claims.
Prescriptions & Renewals
Prescriptions if any are given at the time of service. Patients are able to
call into the office every seven days for refills which are called into their
pharmacy. Patients are instructed to contact their pharmacy after 4:00pm on
the day that they call to get refills.
When surgeries are scheduled, patients are instructed to come to the office
3-4 days prior to the surgery to preadmit. They are to pick up orders at our
office and instructed to go to the hospital for readmission. We precert all
procedures for the patient prior to scheduling.
